Advanced Orthopedics of Oklahoma will conduct sports physicals at Coweta Intermediate High School Tuesday, May 21 from 5-6:30 p.m. for any Coweta student athlete who will compete in sports in the coming school year.
Cost is $15 per person and all proceeds will go to the Coweta Athletic Department to assist with the school’s various sports programs.
School officials remind every athlete needs a physical before they can participate in their respective sport.
“This is a great opportunity to support Coweta Athletics and get the required sports physical out of the way,” Athletic Director Tim Johnson reminded.
Female athletes entering 7th through 12th grades should report at 5 p.m. Male athletes entering the same grades should report at 5:30 p.m. No one will be allowed in to begin a physical after 6:30 p.m.
Johnson said to expedite the process on May 21, athletes should have the OSSAA Physical Examination and Parental Consent Form filled out in advance. He reminded that all forms must be signed by the student’s parent/guardian at the bottom of the form.
The back side of the physical form is for medical professionals to complete.
To obtain a form, go to ossaa.com and click on the Misc Forms link.
